Appeal Process

You have the right to appeal an HGMH decision related to your Freedom of Information request to the Information and Privacy Commissioner of Ontario within 30 days of that decision.

Your appeal letter should be made in writing and include the following:

  • Your name, address and telephone number;
  • A copy of your original request;
  • A copy of the HGMH response;
  • The reasons for your appeal.

The appeal must be accompanied by a fee of $10. if you are appealing a request for access to personal information or $25. if you are appealing a request for access to general records.
